The initial evaluation must be conducted within 45 school days of receiving parental or adult student consent for the evaluation. (USBE SER II.D.2)

Percent of youth with IEPs aged 14 and above with an IEP that includes appropriate measurable postsecondary goals that are annually updated and based upon an age appropriate transition assessment, transition services, including courses of study, that will reasonably enable the student to meet those postsecondary goals, and annual IEP goals related to the student's transition services needs. There also must be evidence that the student was invited to the IEP team meeting where transition services are to be discussed and evidence that, if appropriate, a representative of any participating agency was invited tot he IEP team meeting with the prior consent of the parent or student who has reached the age of majority. (20 U.S.C. 1416(a)(3)(B))

1. LEA selects 20 files of transition age students.
2. State monitors will review the files and identify whether the entire plan meets compliance requirements. One non-compliant item in the transition plan results in a non-compliant plan. 3. Files that are non-compliant at the time of the review will be reported to the federal government through the APR. 4. LEA will make initial corrections within the first 6 weeks to try to avoid written findings of non-compliance. 5. Any files that are not corrected within 6 weeks will need to be corrected within 10 months. Depending on the level of noncompliance of an LEA that could result in a finding of noncompliance. |
