Each time a student needs assistance in the bathroom, it is essential to record that information. The better the records, the more protected everyone involved will be. When a student needs to use the restroom or have a diaper changed, there should always be two staff members present. There should be one person who is directly doing the assisting and another who, is the indirect, ensuring that the process is completed properly. Neither of these two individuals should be the Special Education Teacher in the classroom. Below is an example recording sheet. Under direct and indirect are the initials of the paraeducators or one-on-ones in the classroom. Direct sub and Indirect sub refer to a substitute paraeducator or those who do not have their initials already listed. There should be one form per student so a copy can be shown or sent to parents.
